Transition
Transition is the time from age 14 to
25. It is the time leading up to the point where someone turns 18
and moves from one set of services to another, from children's
services to adult services. It also covers the time of leaving
school.
We have made an information pack to tell people about

There are other organisations that can give advice. One of them is
CXL Connexions who work with young people still at school. They
have a website - www.cx-lancs.org.uk. Their

Darwen.
The 'Transitions Subgroup' is part of the Partnership Board.
The subgroup looks at what's already in place to support young
people as they approach the age of 18. It also comes up with ideas
to make things better.
The Transitions Subgroup has helped to ask for money to pay for
'person centred review' training. This training is now taking
place. It has asked for the money to print an information pack. The
money has been agreed.
The group helped to write and edit the information in the
Transitions Information Pack.
The group is now starting to look at how all those services
involved in transition can work together to make things better for
young people and their families.
